Adoption Application

Thanks for your interest in adopting a rescue dog!

Applicants must live within driving distance of Fort Wayne, Indiana and be willing to bring any current dogs to the foster dog's home for a meet and greet (in Fort Wayne) in order to adopt from us. We require all applicants that have owned a dog to have them up to date on vaccines, be neutered/spayed, and use heart worm preventative regularly. We do not adopt to households using invisible fences, shock collars, prong collars, or choke collars. We do not adopt to homes that allow dogs to roam outside of a fenced area without a leash. We do not adopt to an intermediary; we place dogs directly in their forever homes. Applications are good for 6 months from the date of submission.

A $15 Application Contribution is required when submitting an adoption application. This contribution helps support Heartland Cavalier Connection's (HCC) rescue work, the time to process applications, and the care of dogs awaiting their forever homes. Submitting an application and making the contribution does not guarantee approval, and approval does not guarantee an adoption or placement with a specific dog.

Approved applications remain active for 6 months from the date the application is received. HCC's goal is to find the best match for each individual dog. Dogs are not necessarily placed in the order applications are received. If an approved applicant completes an adoption during the 6 month eligibility period, the full $15 Application Contribution will be credited toward the adoption fee. If an application is declined, the applicant withdraws, asks to be removed from consideration, the application expires, or a suitable match is not identified during the eligibility period, the $15 Adoption Contribution is nonrefundable and remains a donation to Heartland Cavalier Connection. If an application expires after six months, a new application and $15 Application Contribution may be required to be considered for future adoption at the discretion of HCC Board. .

I agree that if at anytime during the animal’s life if I am unwilling or unable to care for the animal, I will contact Heartland Cavalier Connection Inc to manage rehoming or provide care to the dog.

I allow Heartland Cavalier Connection Inc to visit my home to do a scheduled home visit before I adopt the animal, as well as manage an interaction between the adoptable dog and all animals in my household before an adoption takes place.

I allow Heartland Cavalier Connection Inc to contact my veterinarian for a medical reference.

I understand that accidental animal bites or other injuries to humans and other animals do occur, and agree to hold harmless and indemnify, and protect Heartland Cavalier Connection Inc from any claim or suit filed by anyone as a result of such an incident. In addition, our organization will not be responsible if animal should damage or destroy property belonging to me or others, or shall transfer any disease or internal or external parasites to other animals belonging to me or other animals.
